Billing Options

We understand the difficulties associated with healthcare reimbursements, so we would like to help our clients navigate our billing processes and procedures with ease and clarity.  Please continue to read on and learn about each billing option in detail. 

Institutional/Entity Billing Option

Institutional billing is available for all referral laboratories, facilities and institutions. An institutional account is required for all clients. To establish an institutional account, please complete the New Client Setup Form. If you are unsure if you have an existing client account, please contact Venessa Gamboa at venessa.gamboa@uchicagomedicine.org. A completed test requisition form with the “institutional billing" section clearly marked/stamped is necessary in order for testing of sample to be initiated. Samples received with incomplete billing information will delay processing time.

Insurance Option

The University of Chicago Genetics Services Laboratories accepts most insurance plans, with the exception of Illinois and out-of-state Medicaid. For samples being billed to a patient's insurance, prior authorization is required. We do not perform prior authorization services; if insurance billing is desired, the ordering provider must request prior authorization before submitting the sample. If the insurance company does not require prior authorization, please provide a reference number or other documentation that PA was attempted and is not required. All paperwork and required authorization(s) must be submitted at the time of sample submission. Samples received with incomplete billing information will delay processing time.
